It is believed that cognitive behavior therapy (CBT) is an effective treatment of various psychiatric disorders and conditions. However, it was very recent that controlled systematic study of the effectiveness for CBT began. CBT is a definitely promising intervention for various psychological conditions, such as depression, anxiety disorders, somatoform disorders, eating disorders, substance use disorders, schizophrenia, personality disorders, sleep disorders, psychosomatic disorders, and so on, but it is needed to identify effective treatments for particular disorders based on evidence by systematized studies. Firstly, there should be scientific evidence with the goal of measuring the efficacy of any given intervention. In addition, effectiveness including the feasibility and generalizability of the treatments should be tested. This review suggests that CBT might benefit psychiatric patients with various disorders and should be subjected to a controlled treatment trial. These trials should be designed to be both clinically meaningful and widely applicable. By properly evaluating not only the efficacy of CBT, as derived from tightly controlled clinical trials, but also the effectiveness or clinical utility of the intervention in the real practice situation in which it is applied, we can obtain a true picture of the worth of CBT.
